Subject: Medical Diagnostic Equipment (MDE) Accessibility – Compliance with Connecticut General Statutes §19a-490dd
InHouse Medical LLC is committed to providing equitable, high-quality, and accessible care for all patients, including individuals with disabilities. In accordance with the requirements established under Section 19a-490dd of the Connecticut General Statutes, as amended by Public Act 24-113, InHouse Medical has implemented policies and procedures to comply with all state and federal accessibility standards for medical diagnostic equipment (MDE).
Beginning January 1, 2025, InHouse Medical has:
Trained all clinical staff with direct patient care responsibilities on accessibility policies and procedures to ensure that patients with disabilities receive safe, respectful, and appropriate care.
Designated a dedicated contact line for patients who require assistance with access to care or accommodations. Patients may contact our accessibility coordinator at: (203)819-5232
Completed an internal inventory and review of all medical diagnostic equipment to identify which items meet accessibility standards and which require modification or replacement. Documentation of this review, including our action plan for accessibility improvements, is available to the Connecticut Department of Public Health upon request.
In accordance with Section 19a-490dd(e), effective January 1, 2026, InHouse Medical LLC will:
Independently verify or obtain assurance that any newly purchased, leased, or replaced medical diagnostic equipment meets accessibility standards consistent with Section 504 of the Rehabilitation Act of 1973.
Maintain documentation of such verification or assurance as part of our compliance record.
Ensure that at least one examination room contains an accessible exam table or chair designed to accommodate individuals using assistive devices such as wheelchairs.
Maintain an accessible weight scale for patients who require it.
InHouse Medical also supports the use of the Checklist of Best Practices to Provide Health Care to Adults with Disabilities, developed through the Accessible MDE Task Force, as a guide to promote inclusive care and patient dignity during all medical visits.

Telehealth Consent Policy
Effective Date: October 2025
InHouse Medical LLC provides telehealth services to patients located in Connecticut and Massachusetts in accordance with applicable state and federal laws governing the practice of telemedicine.
Our telehealth services are designed to offer convenient, secure, and effective medical care for patients who prefer or require remote visits. By scheduling or participating in a telehealth appointment with InHouse Medical, patients acknowledge and agree to the following:
1. Nature of Telehealth Services
Telehealth involves the use of electronic communications and technology to enable healthcare providers at different locations to share medical information for the purpose of improving patient care. Telehealth may include real-time video conferencing, secure messaging, audio communication, and digital transmission of medical data such as images, lab results, or prescriptions.
2. Patient Eligibility and Location
Telehealth services are available only to patients who are physically located in the states where InHouse Medical providers are licensed to practice. At this time, telehealth visits are offered to patients located in Connecticut and Massachusetts.
Patients must confirm their physical location at the time of each telehealth visit, as state regulations require providers to hold a valid license in the state where the patient is located during the appointment.
3. Risks and Benefits of Telehealth
Potential Benefits:
Increased access to healthcare, including convenient scheduling and reduced travel time.
Continuity of care for established patients through remote follow-ups.
The ability to manage ongoing conditions and preventive health needs efficiently.
Potential Risks:
Technical failures (poor internet connection, platform malfunction) could delay or interrupt care.
Limitations in performing physical examinations may impact diagnostic accuracy.
Security breaches, though unlikely, could occur despite encryption and privacy safeguards.
InHouse Medical utilizes HIPAA-compliant telehealth platforms and encrypted connections to protect patient privacy and minimize these risks.
4. Patient Rights
Patients using telehealth services have the same rights as those receiving in-person care. This includes:
The right to refuse or discontinue telehealth at any time without affecting future care or access to services.
The right to confidentiality of medical information as protected under HIPAA.
The right to know the identity and credentials of the healthcare provider.
The right to request an in-person visit when medically appropriate.
5. Privacy and Security
All telehealth communications are conducted through secure, encrypted platforms that meet or exceed federal privacy and security requirements under HIPAA (Health Insurance Portability and Accountability Act).
No telehealth sessions will be recorded, stored, or shared without the patient’s prior written consent, except as required by law.
6. Consent to Treatment
By initiating a telehealth appointment, the patient acknowledges understanding the information above and voluntarily consents to receive healthcare services via telehealth from an authorized provider of InHouse Medical LLC.
If a patient does not consent to telehealth, alternative in-person appointments are available at our physical office location in Waterbury, Connecticut.
Operational and Safety Information
A. Emergency Disclaimer
InHouse Medical LLC is not an emergency medical provider and does not offer urgent or emergency care services.
If you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency department.
Do not attempt to contact InHouse Medical through email, messaging, or telehealth platforms for emergency situations, as these methods are not monitored 24 hours a day.

InHouse Medical LLC provides both in-office and telehealth services for adult patients seeking medical evaluation, treatment, and ongoing management for a variety of health concerns.
In-Office Services (Waterbury, CT):
Comprehensive medical evaluations and follow-ups
Hormone optimization and testosterone replacement therapy
Weight management programs (including GLP-1 therapy such as Semaglutide and Tirzepatide)
Injectable therapies including Vitamin B12, MIC, L-Carnitine, NAD+, and peptide therapy
Neuromodulators and related wellness services
Physical Exams
Accident/MVA evals
DOT evaluations
Telehealth Services (Connecticut and Massachusetts):
Initial consultations and follow-up visits for hormone optimization and weight management
Review and interpretation of lab results
Medication management for approved therapies under applicable state laws
Education and counseling on treatment options and lifestyle guidance
Telehealth Limitations:
No urgent care visits
No nursing home or assisted living visits
Pediatric care is not provided.
Certain procedures and examinations must be performed in person at our Waterbury, CT office.
